HYBRID STUDENTS (IN-PERSON) IMPORTANT REMINDERS

Did you complete the survey and request in person learning as requested by CPS?

IF NOT, PLEASE DO NOT ATTEND IN PERSON. ALL IN PERSON STUDENTS OPTED IN THROUGH A PHONE CALL, EMAIL OR TEXT IN DECEMBER 2020

Please read below IF YOU ARE APPROVED TO RETURN FEBRUARY 1ST FOR IMPORTANT INFORMATION…

HYBRID STUDENTS (IN-PERSON) IMPORTANT REMINDERS

1. All students must complete the health screener daily before arriving to the school – SEE BELOW FOR INSTRUCTIONS:

Accessing The Student Health Screener – EVERY MORNING – NO EXCEPTIONS

· Every morning, parents and guardians will receive a text message (if they have opted to receive CPS text message notifications) with a unique link to their student’s health screener. These links will have your student’s information pre-populated, offering a quicker way to complete the screener. If your student will be staying with someone else, you can forward the unique student link to them to complete. Students who are 18 years or older may fill out the health screener themselves.

Please contact your school to confirm that they have your most up-to-date contact information. Text messages are sent to the primary contact listed in Aspen and you must opt in to receiving SMS text messages. Standard data rates and charges apply.

· If you have not opted-in for CPS text notifications or do not receive the unique student link for any reason: you can access the daily screener at cps.edu/healthscreener

To complete the screener, you will need to enter the following:

Student ID number or CPS email address

Student Date of Birth

All students must have up to date shots and physicals on file or will be excluded per CPS policy. Parents and guardians have received NUMEROUS reminders regarding compliance since September. To discuss health compliance, please call Ms. McGehee at 773-535-0615 and leave a message. Ms. McGehee will call you ASAP to assist you.

STAY SAFE! KEEP YOUR SOCIAL DISTANCE AT ALL TIMES

  • Maintain at least 6 feet of distance between you and others, even when wearing a mask

    • This will help prevent staff from being deemed “close contacts” and being required to quarantine.

  • Always wear your cloth face covering over your nose and mouth, unless you are eating or drinking.

  • Avoid taking breaks with others in the same room if possible - never eat or drink within 6 feet of others

  • A close contact is someone who has been within 6 feet of someone who tested positive for COVID-19 for more than 15 minutes in a day

Do not be a  “close contact!” - Stay 6 feet away from others to keep yourself and others safe
